ACEs Questionnaire

The ACE Questionnaire comes from the CDC-Kaiser Permanente Adverse Childhood Experiences Study. It counts 10 kinds of hard experiences in childhood, for a score from 0 to 10. What happened to you is not who you are. This helps you see it clearly.

1 Answer 10 questions

Answer Yes or No about your first 18 years of life. You never have to describe what happened.

2 See your ACE score

Each Yes adds one point, for a score from 0 to 10. It counts kinds of experiences, not how often or how severe.

3 Decide on a next step

There is no official cutoff. Research often compares 4 or more ACEs with none. Any score is a reason to be kind to yourself.

What this is — and isn’t. This is a self-check, not a diagnosis. Only a qualified professional can tell you what your results mean for you. If you’re ever in crisis or thinking of harming yourself, call or text 988 (Veterans press 1), text 838255, or call 911.
